Green Department of Paediatric Neurology and the Regional Inherited Metabolic Disease Laboratory, Birmingham Children’s Hospital, Birmingham, UK Sodium valproate (VPA) is an effective anti-epileptic drug (AED) in infancy. However. it has been associated with sometimes fatal adverse effects on liver function. Twenty-two children under 5 years of age prescribed VPA were therefore studied before or shortly after starting treatment and 1 and 3 months later. Plasma VPA, urea, creatinine, albumin, liver enzymes, lactate, ammonia (NHj), free fatty acids, 3-hydxroxybutyrate, glucose, free and total carnitine, quantitative plasma amino acids and urine organic and amino acids were analysed. Mean plasma free camitine was 41 ~mol/l(flO wmol/l) before VPA, falling to 25 pmol/l(&8 ,umol/l) after 3 months (reference range of 15-53 pmol/l); and to below the reference range in one patient. Moderate increases in NH3 (between 50 and 90 pmol/l) were seen in 53% of patients; no IEM was found. Aspartate transaminase was > 50 III/l in 36% of patients with no apparent adverse effects. Two patients showed marked increases in NH3 (99 and 124 lmol/l) with concurrent increases in lactate (5.9 and 4.5 pmol/l respectively) and their alkaline phosphatase (ALP) was > 1000 IU/l. Cytochrome c oxidase deficiency was diagnosed in one, the other died unexpectedly with no diagnosis. Plasma lactate, NH3 and ALP may, therefore, be predictors of a metabolic problem likely to be compounded by VPA.

The Effects of Sodium Valproate in Improving Developmental Delay in Seizure-Free Children with Abnormal Electroencephalography
Background: Developmental delay is one of the most common problems of children referred to pediatric neurology clinics. While there are reports on rehabilitation and its effects, limited studies are available to delineate pharmacotherapy of such children. Because many children with developmental delay have abnormal findings in electroencephalography, we aimed to treat a group of these children,...
متن کاملDoes Chronic Administration of Sodium Valproate to Juvenile Rats Induce Movement Disorder and Cognitive Dysfunction during Adulthood?
Background: Children with seizure disorder are often treated with sodium valproate (SV) on long-term basis. SV acts mainly through gamma amino butyric acid pathways, reducing the excitatory neurotransmission and modifying the monoamine concentration. Altered monoamine concentration by SV is expected to cause movement disorder and cognitive dysfunction, considered reversible after the withdrawal...
متن کاملP 91: The Evaluation of Serum Level of 25-Hydroxy Vitamin D in Patients under Sodium Valproate Medication
Epilepsy is one of the common neurologic disorders in children. Drug side-effects are one of the important problems in treating patients with epilepsy. Using the antiepileptic medication may cause different side effects such as disorder in bone and vitamin D metabolism. Sodium valproate is one of the antiepileptic medications used widely. Some of its side effects include digestive disorder, inc...
متن کاملEffects of Lycopene and Sodium Valproate on Pentylenetetrazol-Induced Kindling in Mice
Sodium valproate and tomato extract have been studied in different experimental models of epilepsy individually. The aim of the present study was to evaluate the effect of lycopene on the antiepileptic effects of sodium valproate against pentylenetetrazol-induced kindling in mice. Swiss albino mice of either sex were randomly divided into 5 groups, with each group containing 8 mice. These group...
متن کاملInvestigation the Effect of Valproate on Locomotor Activity, Balance, Lipid Profile, and Serum Biochemical Parameters before Sexual Maturity in Rat
Introduction: Valproate as an effective antiepileptic drug is used for the treatment of partial and generalized seizure in adults and children. This medication causes biochemical changes in liver functions and behavioral changes in childhood. Therefore, this study evaluated the effects of valproate on locomotor activity, balance, hepatic enzymes, and serum lipid profiles in immature female rats...
متن کاملP-15: Effect of Consumption of Korean Red Ginseng and Sodium Valproate on Apoptosis of Spermatogenic Cells and Sperm Quality in Pilockarpin-Induced Epilepsy Rat Model
Background Reproductive dysfunction and endocrine disorders are common among men with complex partial seizures of temporal lobe origin. Diminished sexual desire and responsiveness along with decreased libido and less orgasm are frequently described in men with temporal lobe epilepsy (TLE). It appears that the reduced fertility in epileptic men is accompanied by erectile dysfunction and abnormal...
